Adoption Application

• Verification of Information: Pawsitive Place Rescue has permission to verify all information provided, including veterinary, housing, and personal references.
• Accuracy of Information: I certify that all information provided is true and complete. False or misleading information may result in denial or revocation of adoption.
• Right of Refusal: The Organization reserves the right to deny any application to ensure the welfare of the animal.
• Financial Responsibility: I understand that I am responsible for all costs associated with the animal, including food, housing, and veterinary care.
• Lifetime Commitment: If I can no longer care for the animal, I agree to return the animal to Pawsitive Place Rescue.
• Adjustment Period: I understand that a new animal requires time to adjust and I agree to be patient during this transition.
• Humane Care: I agree to provide a safe, responsible, and humane environment at all times.
• Follow-Up Contact: I agree that the Organization may contact me regarding the welfare of the animal.
